SURGICAL OPERATIONS UNDER CONTRACT WITH NFZ ( the NATIONAL HEALTH INSURANCE FUND)
LIM MC has made a contract with NFZ for the performance of general surgical operations within the one-off programme for a period from January 1, 2008 to April 30, 2008.
The operations are carried out at the outpatient clinic in ul. Domaniewska 41, in the operating block, which provides excellent facilities for performing such operations.
The following procedures can be performed under the said contract:
-operations on gall bladder - by laparoscopy
-operations on hernia - by classic method
-operations on varicose veins in the lower limbs
-gastroscopy - all kinds of examinations
-colonoscopy - all kinds of examinations
-removing skin changes including histopathology
A patient that needs to undergo an operation within the one-off program is required to present:
1. a doctor's referral to hospital,
The referral should contain a diagnosis and the doctor's recommendation that surgery should be performed within the one-off programme.
2. an ID card or other identity document,
3. a valid insurance card or (optionally):
- a ZUS RMUA - a Monthly Report on a Person Insured - for the month preceding the examination,
- an Employee's Identity Card with a current entry by the employer,
- a Pensioner's Card or the last pension slip,
- a valid Disability Pensioner's Card,
- a certificate - an insurance card or an RMUA - for those insured at KRUS ( the Farmers' Insurance Fund),
- a certificate from a district Job Centre stating that a person is unemployed - valid one month.
