The LUX MED Group has developed a medical care scheme "Senior Sapphire" covering a wide range of specialist examinations and consultations, as well as an attractive discount on services not included in the offer. This medical care scheme is directed to persons aged 65 and over, with no upper age limit.
The offered range includes full-scope medical consultations as part of compulsory prophylaxis for the given age group, full-scope laboratory tests and diagnostic examinations, as well as dental prevention, allergy skin tests, etc. With the Senior "Sapphire" Scheme, you can take advantage of the professional medical care guaranteed by the LUX MED Group.

Terms of joining the individual scheme Senior "SAPPHIRE":

• the offer is directed to persons aged 65 and over (no upper age limit),
• the Contract is signed for the term of at least one year, with a possibility of extending it for another year,
• the payment is made in half-yearly instalments or upfront for the full year to the bank account indicated in the Contract
• the package is valid at all facilities owned by the LUX MED Group (LUX MED, CM LIM Medycyna Rodzinna, PROMEDIS),
• two copies of the Contract, signed on behalf of LUX MED, are sent to the Client for his/her signature, by priority mail to the indicated mailing address,
• the Contract becomes effective as of the 1st day of the following month, upon receiving by the Sales Department, at least 5 working days before the given month's end, of confirmation of a payment being made to the indicated bank account and the original of signed Contract.
